Patient Safety Incident Response Framework (PSIRF)

Following the publication of the National Patient Safety Strategy in 2019, the Patient Safety Incident Response Framework was published in August 2022 with acute providers expected to implement the framework by September 2023.

PSIRF can only be successful if it is considered a complete transformation programme grounded in human factors and continuous quality improvement. Organisations that merely tweak the serious incident framework will not achieve a reduction in patient harm or improvements in its organisational safety culture.
